Deliver patient‑centered care to approximately 1,700 male and female inmates, providing primary care, chronic disease management, and health education.
Role
As a Registered Nurse – Chronic Disease, you will identify, manage, and support patients with chronic conditions, conduct assessments, develop care plans, coordinate referrals, and deliver health education. You will work closely with a multidisciplinary team to ensure evidence‑based and culturally appropriate care.
