Location : Hervey Bay | Mon–Fri, 8 : 00am – 4 : 30pm

Make a real difference in your local community. Just Better Care is seeking a dedicated Clinical or Non-Clinical Care Partner to join our Hervey Bay team. In this community-based role, you’ll provide personalised support to older Australians, helping them live safely and independently at home.

Key Responsibilities

• Complete in-home assessments and care planning
• Manage participant cases and provide clinical / holistic support
• Track participant wellbeing and document progress and goals
• Liaise with medical professionals, allied health, and service providers
• Maintain documentation to ensure compliance (CRM, funding, evidence uploads)
• Oversee participant budgets and ensure funding guidelines are met
• Support training and guidance of support workers and nurses
• Schedule and coordinate services, including urgent changes
• Contribute to ongoing audits, reporting, and professional development
• (Clinical Care Partners only) Provide occasional wound care
