About the role
The Discharge Patient Flow Concierge is responsible for providing administrative support for discharge planning and assisting patients and their families with the process of leaving the hospital and transitioning back to their home or other care setting.
Supporting discharge processes in collaboration with the multidisciplinary team to expedite timely discharge and facilitate integration with out‑of‑care hospital and primary care services where appropriate. Facilitating and enhancing communication between the patient, families, carers, clinical staff, out‑of‑hospital care services and primary care to improve the discharge experience for patients, carers and families.
