This full-time, office-based role (Monday to Friday, 9 : 00 AM – 5 : 00 PM) offers a supportive, collaborative environment with leaders who genuinely care and back their team.
About the Role
As a Care Coordinator, you’ll be responsible for coordinating services and supports for clients receiving Home Care Packages. You’ll build meaningful relationships with clients, their families, and service providers to ensure that care plans are person-centred, effective, and compliant.
You’ll be the key contact for your client portfolio — managing budgets, scheduling services, and helping clients understand their package spend and available choices.
