The service provides quality professional community nursing care and uses a progressive model of service delivery to promote accessibility, patient autonomy and measurable health outcomes.

What you\'ll be doing
Assess, plan, implement and evaluate community nursing care, in the patient home or clinic, in collaboration with individuals and the broader health care team to achieve quality patient outcomes. The service provides generalist community nursing to clients in their homes or clinic environments, including : comprehensive assessment, referrals to other services and specialists, medication administration (oral, parenteral, topical, etc.), wound management (acute to complex, including compression therapy, NPWT, pressure injury management, conservative sharp wound debridement and vascular profiling), palliative and end-of-life care, syringe driver management, respiratory / heart failure monitoring, IDC / SPC changes and support, continence assessments, CVAD care, and more.
