What you’ll be doing :
Respond assertively to the needs of children, caregivers and families at risk of poor outcomes. Provide intensive support to those with additional needs, targeting actions and interventions to change the experience and life trajectory of these children and families.Provide a flexible, place‑based outreach service focused on improving outcomes for infants, children, caregivers and families who are experiencing increased need.Actively engage with internal and external partners to support children and families in settings such as client homes, Hume’s mobile MCH van, preschools, childcare services and Primary School Hubs.Ensure early contact of vulnerable clients, working alongside existing enhanced and maternity services where appropriate.Ensure each child receives the recommended schedule of health monitoring activities from birth to 6 years at key development ages and stages (KAS visits), as specified by the Department of Health.Intervene early to address developmental or family issues before they escalate.The skills and experience you’ll bring to the role :
Knowledge of well‑child development, deviations from normal, and relevant assessment skills.Knowledge of post‑natal recovery of the mother and relevant assessment skills.Knowledge of family dynamics and its impact on the child.Good communication and writing skills, including ability to gain cooperation from diverse backgrounds.Demonstrated ability to work as a sole practitioner within a multi‑disciplinary team.Computer literacy.Required Certifications & Qualifications :
Registered Nurse and Midwife with the AHPRA, and an accredited post‑graduate qualification in Maternal and Child Health nursing.Tertiary qualifications in a related discipline and / or appropriate experience.Current driver’s licence and Working with Children Check.Why Hume City Council?
