About The Opportunity

Your duties will include, but are not limited to :

• Providing care provision in accordance with residents’ individualized assessed care needs.
• Ensuring resident care is within the individualized care plan or as directed by RN and / or Care Manager.
• Reporting all incidents in a timely manner to the Registered Nurse or Care Manager.
• Completing all activities relating to Residents Care documentation plans.
• Timely notification of changes in resident care to RN in charge and / or Care Manager.
• Ensuring resident care is within the AIN scope of practice and as per Bolton Clarke Quality Management Committee.

The Successful Candidate Must Demonstrate The Following
