About the Role :
The Critical Care Liaison Nurse (CCLN) is responsible for the comprehensive follow-up review and assessment of patients discharged from the ICU; the facilitation of timely admission / re-admission to the ICU if appropriate; the sharing of critical care skills with ward based multidisciplinary staff and contributing to the development and improvement of critical care services with St Vincents.
The CCLN is an integral part of the Critical Care Liaison Service which is based in the ICU. Each CCLN actively contributes to the Tracheostomy Review Service and the Medical Emergency Team (MET).
The Critical Care Liaison service has seven basic aims :
- to review all patients discharged from the Intensive Care Unit
- to provide consultation to ward staff regarding treatment and management of patients
- to aid timely review and care planning of critically ill patients in the Emergency Department (ED)
- to share clinical skills to enhance knowledge of ward staff
- to avert re-admissions to the ICU or to facilitate early and appropriate admission
- to improve communication between ICU and the acute wards
- in conjunction with the ICU Associate Nurse Unit Manager (ANUM) support the operational needs of the ICU
This challenging yet very rewarding role within the hospital will enable you to share your clinical experience and knowledge. As the Critical Care Liaison Nurse you will be : part of the Medical Emergency Team Response; responsible for the comprehensive follow-up review and assessment of patients discharged from the ICU; facilitating timely admission / re-admission to the ICU; sharing your critical care skills with ward based multidisciplinary staff and contributing to the development and improvement of critical care services within St Vincents.
This role is offered as a permanent night duty position on the basis of 10 hour shifts.
Classification for this position is ZJ4 (Grade CAPR 3.2) ($122907.20 per annum).

About SVHM - Intensive Care Unit
The Intensive Care Unit (ICU) at St Vincents Hospital Melbourne (SVHM) is a 19 bed environment (funded to 15 ICU equivalent beds) caring for patients requiring critical care. It accommodates elective and emergency surgical and medical patients. The ICU admits deteriorating patients from the ward environment Operating Theatre Emergency Department plus external admissions from other metropolitan and rural hospitals via Adult Retrieval Victoria (ARV).
The ICU maintains the Medical Emergency Team Critical Care Liaison Nurse (CCLN) Team and Tracheostomy Review Service. The Department is a tertiary referral centre for Neurosurgery and Cardiothoracic surgery and is a Level 3 VECMOS Centre.

RESPONSIBILITY AREA 1 : Patient Care service provision
In conjunction with the ICU the CCLN will promote a multidisciplinary approach and ensure the delivery of the highest standards of patient care. This will encompass and promote the mission and values of the Sisters of Charity.
