You will conduct comprehensive assessments, develop person-centred action plans, and provide tailored interventions aimed at reducing preventable hospital admissions and improving health and wellbeing outcomes.
You'll work collaboratively with clients, families, carers, Support Coordinators, and other providers to ensure integrated, responsive care across multiple service sectors.In addition to your clinical responsibilities, you will provide supervision and support to a team of six clinicians, fostering a culture of professional growth and high standards of care.
Your role includes overseeing access to services, facilitating group programs, and advocating for clients across health and social service networks.
