• A leading community care provider is currently seeking a Case Manager to with aged care experience to join their dedicated home care team in Northern Beaches.

• Certificate IV in Aged Care / Community Services or equivalent (minimum)
• Previous experience in Home Care / Community / Aged Care
• Current driver's license
