  • Job Vacancy : Full-time / Part-time / Casual Support Coordinator
  • Due to business expansion, we are hiring a Support Coordinator for NDIS and aged community care.

    • About the Role
    • At Pain Free, Support Coordinator assess needs and coordinate support for NDIS participants and My Aged Care Home Care Package (HCP) clients for Support Coordination services.

      Your key responsibilities as a Support Coordinator includes :

    • Assist NDIS participants and HCP clients to implement plans and coordinate their services and supports.
    • Complete all necessary administration to the required standard, ensuring full compliance.
    • Maintain and develop relations with government bodies and disability associations / groups.
    • Take ownership for developing and retaining these networks.
    • To promote Pain Free to the community and generate interest in support coordination under the NDIS and HCP. This will involve delivery of presentations to various organisation's such as Local Area Coordinators, Disability Service Providers, service providers and support groups including participants, clients and nominees.

    • You'll also need to :

    • Have Minimum 6 months experience working as a Support Coordinator
    • Have Allied Health qualifications; or minimum Certificate IV in Allied Health / Disability / Age care or other relevant community services
    • If you don't have the requirements above, then have a plan to acquire the necessary skills, experience, or qualification and commit to the plan while working on the job
    • Be based in Brisbane, QLD
    • Successful applicants will be required to have_
    • Valid drivers' license, CPR / First Aid Certificates, Police Check, Blue Card and NDIS Worker Screening Check
