• Key Responsibilities

As a Care Coordinator, you will :

• Recruit, train, and supervise a team of carers to ensure the delivery of exceptional care services.
• Coordinate and schedule operational activities in accordance with established care plans and organizational policies.
• Liaise between clients, families, referral sources, and administrative staff to facilitate effective communication.
• Build strong relationships with clients and their families, ensuring a positive and professional experience.
• Educate clients and families on available services and benefits of care plans.
• Monitor and assess the quality of care provided, identifying areas for improvement, and implementing changes as needed.
• Assist with inside sales enquiries and contribute to business development opportunities.
• Ensure compliance with regulatory agencies and safety standards.
• Qualifications and Skills
• Enrolled Nurse
• Knowledge of the aged care industry and related issues.
• Proven ability to assess, plan, implement, and evaluate care services.
• Strong interpersonal and communication skills, both written and oral.
• Computer literacy and analytical abilities.
• Commitment to the Aged Care Sector and the well-being of seniors and vulnerable individuals.
• Driver's License and comprehensively insured private vehicle.
