- Key Responsibilities
As a Care Coordinator, you will :
- Recruit, train, and supervise a team of carers to ensure the delivery of exceptional care services.
- Coordinate and schedule operational activities in accordance with established care plans and organizational policies.
- Liaise between clients, families, referral sources, and administrative staff to facilitate effective communication.
- Build strong relationships with clients and their families, ensuring a positive and professional experience.
- Educate clients and families on available services and benefits of care plans.
- Monitor and assess the quality of care provided, identifying areas for improvement, and implementing changes as needed.
- Assist with inside sales enquiries and contribute to business development opportunities.
- Ensure compliance with regulatory agencies and safety standards.
- Qualifications and Skills
- Enrolled Nurse
- Knowledge of the aged care industry and related issues.
- Proven ability to assess, plan, implement, and evaluate care services.
- Strong interpersonal and communication skills, both written and oral.
- Computer literacy and analytical abilities.
- Commitment to the Aged Care Sector and the well-being of seniors and vulnerable individuals.
- Driver's License and comprehensively insured private vehicle.
