You will play a pivotal role in delivering high-quality care and support to our diverse patient population.

Your responsibilities will encompass, but are not limited to : Conducting patient assessments, creating care plans, and health screenings with a focus on GPMP, TCA, 715, and 75+ health assessments.Administering medications and vaccinations.Performing chronic disease management, including monitoring patients with diabetes, hypertension, and other chronic conditions.Maintaining accurate patient records and assisting with data collection for quality improvement initiatives.Supporting GPs and collaborating with administration and management to ensure efficient clinic operations and delivery of high-quality patient care.Requirements : Registered Nurse qualification with current registration with AHPRA.Experience as a Practice Nurse, preferably in a primary care or general practice setting (not essential).Experience in general practice and with Best Practice software is an advantage (not essential).Understanding of Medicare billing processes and experience in care plan reviews and health assessments.Commitment to providing compassionate, patient-centered care and contributing to the success of the practice.We offer a supportive work environment with a focus on professional development and career growth, as well as flexible work arrangements to support work-life balance.If you are passionate about making a difference in the lives of patients and are committed to providing high-quality primary healthcare services, we invite you to apply for this rewarding opportunity.
