We are seeking
- NDIS SERVICE COORDINATOR
- with the following attributes to join our team :
- Develop individual support plans which are goal focused and include strategies for person centred active support
- Act as a point of contact for enquires and referrals received to undertake intake, support, information and referral of eligible NDIS clients
- Maintain legal and ethical standards which reflect relevant legislation and NDIS guidelines.
- Ensure all participant records are up-to-date, with signed quotes, service agreements etc;
- Attend meetings and regular in person contact with our participants and support worker teams
- Ensuring all documentation is completed to meet relevant standards and audit requirements
- Participation in on call roster on a rotational basis
Skills and experience
To be successful in the role you will need :
- Relevant qualification or industry experience (eg. Degree in Human Services, Cert IV in Disability Services)
- Extensive knowledge and experience of the National Disability Insurance Scheme and who the changes effective both clients and organisations
- Demonstrate commitment and passion to providing excellent customer service
- Ability to effectively communicate with a variety of stakeholders
- Good knowledge of mainstream services, community groups and community resources and community networks
- Demonstrated commitment to quality outcomes and strong client service
- Ability to prioritise workload and work independently and as part of a team
- Experience in - Behaviour Support and Restrictive Practices, Mental Health & Trauma Informed Practice
- Mandatory Onboarding Documents
- Certificate III in Disability Services (or at least 12 months of relevant experience).
- NDIS Worker Screening Check.
- Completion of the NDIS Orientation Module (_NDIS Quality, Safety & You_).
- Working With Children Check (_for underage participants only_).
- Current Driver's License.
- Comprehensive Car Insurance and a reliable vehicle.
- Additional Skillsets
- Experience in personal care and manual handling.
- Behavioral management expertise.
