About The Opportunity

We are seeking a dedicated and compassionate Aboriginal Health Worker to join our team. In this vital role, you will provide culturally safe, patient-centred care coordination to Aboriginal patients with chronic or complex health care needs.

Working in close partnership with clinicians, you will support patients and families through :

  • Providing comprehensive assessment of health and social needs, in collaboration with other health professionals
  • Providing care coordination to ensure seamless access to service
  • Providing education and advocacy to empower individuals and communities

What You'll Be Doing

The position is responsible for the provision of culturally safe, patient centred care coordination services to Aboriginal patients and their families with chronic and / or complex needs.

The position will work collaboratively with a clinician to provide assessment, care coordination, and education to patients, families and carers that takes into account clinical, psychosocial and cultural needs.

Where You'll Be Working

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.

These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.

With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

  • This is an Identified Aboriginal / Torres Strait Islander Position. Applicants for this position must be of Aboriginal and / or Torres Strait Islander descent through parentage, identify as an Aboriginal person and / or Torres Strait Islander and be accepted in the community as such. Exemption is claimed under Section 14 of the Anti-Discrimination Act 1977 (NSW).
  • Certificate III in Aboriginal and Torres Strait Islander Primary Health Care or relevant qualification or willingness to complete and to undertake further training as identified.
  • Demonstrated understanding of the range of factors that have influenced the health status of Aboriginal communities.
  • Demonstrated experience delivering culturally safe health services to Aboriginal communities.
  • Demonstrated written and verbal communication skills and computer literacy.
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
