About the role Building strong connections with clients and their families Assigning staff to clients based on staff qualifications and availability, as well as client preferences Organise support to clients with disabilities and monitoring and reporting on their progress Referring clients to agencies that can provide additional help Assessing client needs and resources for health, welfare and other facilities and services Responding promptly to emails, enquiries and requests from clients, staff and management Maintaining accurate records of incidents and feedback as required Re-organising work priorities to meet the needs of a fast-paced environment Supporting all clients to meet their goals Conducting Client Care Assessments and Care Planning Providing clinical guidance to other team members for client management Discussing and promoting Right at Homes services with medical professions and referral sources Ability to work in a fast-paced, dynamic environment Current AHPRA registration as a Registered Nurse Experienced in assessing and planning care needs Understanding of the Aged Care and Disability systems A strong client focus which is based on their goals, preferences and needs Current National Police Check (or commitment to obtain prior to start) NSW Working with Children Check (or commitment to obtain prior to start) NDIS Worker Screening Check (or commitment to obtain prior to start) NSW Full Drivers Licence All candidates must have the right to work in Australia Organisational, administrative and time management skills Check us out at
