- Support Coordinator
- to join our team on a part-time basis.
- About the Role
This position involves working directly with participants to help them understand and implement their NDIS plans. You'll build relationships, coordinate services, and ensure participants are supported to achieve their goals.
Assisting clients to access and coordinate a range of support services, including allied health, personal care, and social / community engagement activitiesDeveloping and regularly reviewing individualised support plans to ensure clients' evolving needs are metLiaising with service providers, government agencies and other stakeholders to advocate for clients and facilitate effective service deliveryResearching, coordinating and managing a range of supports to suit individual needs across multiple providersMonitoring clients' progress and wellbeing, and identifying opportunities to enhance their independence and quality of lifeGenerating reportsMaintaining detailed shift notes and contributing to the continuous improvement of support coordination processesSkills & QualificationsPrevious experience in NDIS support coordinationStrong knowledge of the NDIS and person centred planningNDIS Worker Screening CheckAbility to work independently and manage multiple participantsExceptional communication and problem-solving skillsCert IV in Community Services, Social Work, or a related field (or relevant experience)Valid Driver's LicenceOwn car and willingness to travel when requiredWhat We OfferFlexible part-time hours to suit your lifestyleSupportive and collaborative team cultureOngoing training and professional developmentOpportunity to make a real impact in people's livesIf you're passionate about supporting people to live their best lives and want to be part of a purpose-driven team, we'd love to hear from you.
