We're looking for an experienced Care Coordinator to provide person-centred virtual care to our members navigating complex healthcare journeys. You'll be the trusted point of contact, helping members access the right services, manage their care needs, improve their self-management and overall wellbeing.

Key Responsibilities Provide one-on-one telephonic care coordination for members with chronic or complex medical conditions Conduct initial assessments and develop tailored care plans Collaborate with healthcare providers, hospitals, and allied health professionals Monitor progress, offer guidance, and adapt care plans as needed Educate and empower members to make informed health decisions Ensure smooth transitions between different levels of care (e.g. hospital to home) 26th June, 2025 About the Role
