Position ObjectivesWe are currently seeking experienced case managers to join the team in the Sydney office on a Permanent Full-time basis.
As a case manager, you will be responsible for delivering case management to HCP consumers that are person-centred and enablement-focused. You will be involved in all aspects of care management and care coordination, including administrative tasks and rostering of client services.
You will develop positive working relationships within the local service centre, and across the organisation to facilitate improved outcomes for our consumers. In this role, you will act by our client policies and procedures, program guidelines, the National Standards of Practice for Case Management and the National Code of Ethics.
You will also be expected to contribute to the organisation's strategic direction, including input and advice to support organisational strategy and objectives.
Key ResponsibilityYou will be supporting vulnerable people in the community through home care packages and case management.Knowledge of aged care sectors, including funding and policy protocols, quality standards and HCP program processes.Capacity to interpret and work within the relevant Policy guidelinesEnsure consumers exercise choice in decision-making about their care & the way services are delivered.Act as the key contact person and advocate for consumers and their carers.Maintain regular communication with clients, carers and their familiesContinually monitor and review consumers' needs and ensure service provision is compliant with the HCP guidelines.Ensure care is delivered safely and by the consumers' needs and goalsArrange and coordinate care plan and support servicesImplement home care quality standards with a compliance officerProvide information, assessment, referral, case management and monitoring services to frail aged people, people with dementia, people with a disability and their carers living at homeWork with older people to enable them to make informed choices about their lifestyle, what is most important to them in terms of identified goals, and to enhance their capacity to live independently in the communityOutsource and build relationships with external providers to meet consumer needs and servicesKnowledge of the needs of diverse groups, such as people from CALD and ATSI backgrounds, Veterans, and the ability to respond sensitively to the unique needs of individuals.Demonstrated capacity to work collaboratively within a multi-skilled team using advanced interpersonal, communication and client service skills.About YouWith experience in Case management, you deliver quality, consumer-focused care. Your ability to build strong relationships is second to none, and you know various funding sources in the aged care sector.
