As part of the ForHealth Group, we're focused on innovation, culture, and exceptional patient care.The RoleWe are looking for an experienced CDM (Chronic Disease Management) Nurse, Enrolled Or Registered Nurse to join our Tuggeranong Medical Centre to be the driving force and advocate for the local community in preventative health.
This role will see you support GPs and their patients to identify and manage their chronic conditions and ultimately keep them out of hospital.
Preferably full time but happy to discuss part time options with the right candidate.The TasksIn this role, you will be involved in a wide variety of tasks including, but not limited to : Prepare Care Plans and Team Care Arrangements to help patients manage their disease / sEmpower patients to increase ownership over their healthCoach patients on embedding health behavior change principlesEnable patients to access ongoing care from a multi-disciplinary teamAssist in early identification of chronic disease by conducting health assessmentsWork with GPs to promote preventative healthcare throughout the practiceIncrease the number of patients who receive a care plan for the first timeEnsure patient's care plans are reviewed regularlyManage administrative tasks relating to carrying out CDM appointmentThere is an opportunity for bonusesThe Successful CandidateWe're looking for nurses who are passionate about patient care and committed to working as part of a collaborative clinical team.
