About the Role :
- Conduct assessments to evaluate clients' needs and abilities.
- Create tailored rehabilitation plans with achievable goals.
- Help clients overcome barriers to returning to work.
- Provide mental health support to manage conditions.
- Collaborate with healthcare providers, employers, and stakeholders.
- Guide clients in exploring new career paths and job opportunities.
- Educate clients and families about the rehabilitation process.
- Track progress, update plans, and maintain accurate records.
- Provide crisis support and connect clients to emergency services when needed.
