• Ensuring appropriate delivery of quality podiatric services tailored to the individual needs of the client and using a person-centred approach
• Holistic podiatric assessment and management of patients / clients in a community setting ie in their own home.
• Interventions provided include : oRoutine nail and skin careoDiabetic foot careoManagement of foot ulcers, fungal nails, plantar warts, neuromasoFootwear assessment and recommendations
• Record appropriate clinical notes for each patient / clientusing the provided online system, in a recognised format.
• Liaise with other Allied Health Professionals, GPs, Case Managers and other Health Care Professionals, as required, within and outside our own team, to help provide holistic care to patients
• Communicate effectively with case managers and other stakeholders, where funding is provided by a third party provider.
