About the Role : As a Care Assessment Coordinator, you will play a vital role in coordinating the AN-ACC (Australian National Aged Care Classification) process, leading pre-admission assessments, and ensuring optimal funding outcomes. You'll work closely with our Residential Manager and Funding Consultants to support residents' care journeys and uphold the highest standards of aged care.

About You : To be considered, you will have :

A qualification as a Registered Nurse, along with experience in aged care and knowledge of the AN-ACCStrong interpersonal and communication skillsProficiency in MS Office and clinical documentation systemsAbility to lead, train, and support staffExcellent time management and problem-solving skillsExperience with ACFI and quality improvement tools - desirableIf you're ready to take the next step in your aged care career and contribute to a team that values compassion, excellence, and integrity, we'd love to hear from you.
