Job descriptionProvision of mental health assessments, including completion of mental state examination, risk assessment, case formulations and an initial treatment plan. Work collaboratively with clients, their families / significant others and relevant service systems to respond appropriately to their mental health needs. Provide timely crisis intervention and work closely with community services and ED's to implement adequate safety planning and goal setting. Participate in triaging / intake of new referrals to the service; establishing what mental health concerns are present, identifying risk factors, current supports available and negotiation of appropriate plans for each person referred to the service. Manage a full case load dependent on acuity of clients. Identify and establish new relationships in at risk communities for the program. Create opportunities for community members to be heard and listened to attentively and encourage them to express their views in relation to the program. Provide both face-to-face consultations and telehealth consultations with clients as required. Provide clients with up-to-date information and education whilst ensuring a positive experience. Clinical service delivery may include medication management and clinical observations. Participate in quality improvement activities designed to ensure that all functions of this role meet Safety & Quality Management Standards. Record incidents and near-misses in line with the protocols, including escalation in accordance with the Severity Assessment Classification (SAC). Engage other relevant services and providers to create professional relationships to ensure positive referral process, continuity of care and open disclosure on concerns including mental health services, NDIS services, aged care homes, LHDs, PHNs, GPs, Practice nurses, schools, community nursing and psychologist / counsellors / psychiatrists. Identify potential risks and manage appropriately in accordance with the policies and procedures. MUST be located around the Murrumbidgee NSW. Current registration as a Registered Nurse / EEN with the Australian Health Practitioner Regulation Agency. Tertiary education in Bachelor of Nursing or Diploma of Nursing. Ability to maintain and instill high levels of confidentiality and professionalism. Demonstrate excellent interpersonal and communication skills, both written and verbal. Ability to work flexibly and independently. Demonstrates advanced clinical assessment skills and care management and crisis management skills. Demonstrates ability to manage a caseload and manage time wisely. Demonstrates ability to work as part of a multidisciplinary team and collaborate to case manage clients. Ability to work collaboratively and display THCFA values. Be fully vaccinated as per the NIP including or willingness to obtain. Current Working with Children Check or willingness to obtain. Current Criminal History Check or willingness to obtain. Someone willing to travel to locations to manage clients in the community. Be self-motivated and able to work autonomously and as part of a team. Have a strong passion for quality care and customer service. Be outgoing and able to engage with the local community and communicate with diverse individuals. Exhibit excellent problem-solving & time management skills. About The Healthy Communities Foundation Australia
The Healthy Communities Foundation Australia was founded by healthcare professionals to change health outcomes in rural and remote communities. Formerly known as Rural and Remote Medical Services, we have been helping Australian communities for over 20 years.
Our Mental Health team is focused on providing quality care to our targeted communities under our Suicide Prevention strategy grant funded by the Department of Health and Suicide Prevention Australia. Our aim is to provide timely access to mental health services, crisis services and assist clients to stay in their community with a focus on both wellness and mental health.
About the Role
As a Nurse with our Mental Health and Suicide Prevention Team your focus will be on delivering excellence in primary health care services and Mental Health services through a commitment to meeting the complex health needs of our targeted communities in Narrandera, Leeton, Darlington Point, Hillston, Hay and other surrounding communities.
