Role Details
The Social Worker is a role within Safer Family Services and isaccountable to the Supervisor for :
Key outcomes and accountabilities :
1. Deliver quality client focused support services to eligibleinfants, children and their families that are identified throughtriage and assessment processes, as having high and complex needsand adhere to practice guidelines, protocols, audits and qualityindicators.
2. Provide a case work response to infants, children and familiesas directed by senior staff, by utilising a comprehensive casemanagement approach including assessments, in partnership withinfants, children, families, colleagues and other services, and arange of intervention approaches to engage children and families,inclusive of, but not limited to infant / child and family centred,strength based, culturally relevant and informed by evidence baseand practice wisdom.
3. Develop practice approaches with infants / children and theirfamilies to determine the goals, focus of service engagement anddelivery and support sustained intervention to vulnerable childrenand families, operating as a member of a multidisciplinary teamutilising an interdisciplinary and inter-agency teamworkapproach.
4. Ensure case management tasks are completed within the KPItimeframe by applying flexible service delivery approaches torespond to individual infants / child and family circumstances andneeds and participating in discussions and processes to manage thelevel of care and support required for children and their families,including case conferencing.
5. Assess the safety of, and negotiate the mitigation of risk to,infants / children including communicating effectively andefficiently with Supervisor in relation to issues of escalation forsafety and care.
6. Contribute to the maintenance of data and record management toimprove service delivery including maintaining high quality casenotes, ensuring that all relevant client data is completedaccurately and entered into the client management system, managingclient files and documentation in accordance with agency policy andpractices and participating in case reviews and file audits.
7. Continue to maintain and develop skills and expertise includingparticipating in regular supervision and cultural and otherspecialist consultations, undertaking and evidencing continuingprofessional development activities, and participating in workplacetraining focused on upgrading skills and knowledge.
Note : Any other responsibilities in line with the classificationlevel of the role as assigned by the Supervisor and / or theDepartment. The responsibilities as specified above may be alteredin accordance with the changing requirements of the role.
