Clinical Partner | Northern Sydney – HammondCare
HammondCare at Home is looking for an experienced Clinical Partner to support the team in Northern Sydney. The Clinical Partner delivers high-quality, person-centered clinical care that supports clients to live well in their own home. This role focuses on assessing clinical care needs, developing and reviewing care plans, and delivering clinical care in partnership with clients and key stakeholders. The position works with the care management team to ensure the best outcomes for clients in the community and provides guidance to care staff to ensure safe, continuous delivery of high-quality clinical care.
Responsibilities
- Assess clinical care needs and develop, implement, and review care plans in collaboration with clients and families.
- Deliver evidence-based, best-practice clinical care in a home-based setting.
- Collaborate with the care management team to achieve optimal client outcomes across multiple sites.
- Provide guidance and supervision to care staff to ensure safe and continuous service delivery.
- Engage in multidisciplinary teamwork and ensure alignment with HammondCare’s Mission in Action.
Qualifications
Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).Commitment to client-focused service delivery in aged and community / home care settings.Knowledge of Aged Care Reforms and Aged Care Quality Standards.Excellent communication, collaboration, advocacy, and person-centred care skills.Ability to work effectively in a multidisciplinary team and independently across multiple sites.Current driver’s license and ability to travel between HammondCare sites and services.Strong administrative and computer skills, including digital health records and care coordination tools.Commitment to professional development and empathy, patience, and passion for high-quality, person-centred care.Desirable
Nursing experience in aged care and community care.Ability to identify and implement innovations in care delivery and service improvement.About HammondCare
