About The Role

We are looking for Youth Lead Practitioners to support adolescents and young adults aged 10–24 as they navigate life with a disability, from adolescence through to early adulthood.

As a Youth Lead Practitioner At Everyday Independence, No Two Days Will Be The Same. If You Were Here Last Week, These Are Some Of The Things You May Have Done

• Led the interdisciplinary team that plans a young person’s NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
• Utilised assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
• Worked within the social model which means you’ll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
• Held meaningful, future-focused conversations with participants and their supports
• Empowered participants and their families to develop habits and routines
• Built meaningful relationships with your participants and their supports (family, carers, friends)
• Documented, and billed for, each session through our online participant management system
• Attended the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs
• You’ll have met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and / or support
