Responsibilities
Case Management for Homeless Youth : Provide comprehensive case management support to young people aged 16-24 who are homeless or at risk of homelessness. Assess needs, develop personalized support plans, and connect them with appropriate resources and services.Program Delivery : Deliver a range of support services within two programs : Creating Connections and Transitional Support. Help young people achieve suitable housing outcomes and enhance life and living skills.Collaboration with Team : Work closely with the Youth Housing Supports team to ensure young people receive the necessary support. Share information, coordinate services, and advocate for their needs.Adherence to Guidelines and Policies : Be knowledgeable about and adhere to program guidelines, organisational policies, and procedures to ensure consistent, ethical, and effective support.Youth-Focused Approach : Commit to working with young people in a supportive and empowering way. Build rapport, listen to their needs, and help them develop skills and confidence to succeed.The Ideal Candidate will bring
Empathy and Understanding : Deep understanding of challenges faced by young people in housing situations. Empathy is crucial for building rapport, providing effective support, and advocating for their needs.Strong Communication Skills : Ability to build relationships with young people, collaborate with partner agencies, and advocate for their needs. Communicate clearly, listen actively, and adapt communication style as needed.Organisational and Time Management Skills : Manage multiple cases, coordinate with various agencies, and maintain accurate records.
