About the Role

As a Family Support Practitioner, you'll deliver vital outreach family case management to vulnerable and marginalised families, caregivers, children, and young people in Brisbane’s inner northern suburbs.

This full-time opportunity supports our multidisciplinary team approach, helping families at risk with information, practical skill development, and intensive support strategies.

What You'll Do

· Engage with families, build trust, and obtain consent for support.

· Develop and implement case plans that correspond to identified unmet needs and child protection concerns.

· Work collaboratively with a wide range of services and professionals.

· Advocate for families and support them through complex challenges.

· Build culturally safe, respectful, and inclusive relationships with families from diverse backgrounds.

What You'll Bring

· A relevant tertiary qualification in Human Services or related discipline.

· Demonstrated experience working with families experiencing high levels of challenges.

· High cultural competency with the ability to work sensitively across cultures.

· Strong collaboration and communication skills.

· A passion for social justice and making a difference in the lives of children and families.
