Local Program

Sacred Heart Local (SHL) is a vital in‑home support program within Sacred Heart Mission, delivering compassionate, person‑centred care to older people who are at risk of homelessness or social disadvantage. Through the Support at Home (SaH) and Commonwealth Home Support Program (CHSP) , SHL empowers clients to live independently and with dignity in their own homes and communities.

Position Summary

Location : Melbourne, VIC

Classification : Registered Nurse Level 4 (as per Sacred Heart Mission Nurses’ Enterprise Agreement 2019 or subsequent Agreements)

Employment Type : Full‑time

Reports to : Manager, Sacred Heart Local

Supervises : Care Partners & Support Workers

We are seeking a passionate and experienced Clinical Team Leader to provide clinical oversight, leadership, and support to our dedicated team of Care Partners and Support Workers. This role is pivotal in ensuring the delivery of high‑quality, safe, and responsive care aligned with the Aged Care Quality Standards and the values of Sacred Heart Mission.

Day‑to‑Day Responsibilities

  • Provide clinical leadership and day‑to‑day oversight of care delivery under the Support at Home Program
  • Supervise and mentor Care Partners and Support Workers, fostering a collaborative and respectful team culture
  • Conduct clinical assessments and develop care plans for clients with complex needs
  • Monitor clinical risks, lead case conferences, and ensure compliance with aged care standards
  • Drive continuous quality improvement through audits, data analysis, and reflective practice
  • Advocate for client rights and ensure care is person‑centred, culturally sensitive, and aligned with individual goals
  • Maintain accurate clinical documentation and ensure compliance with My Aged Care and SHM policies

Qualifications

  • Registered Nurse (Level 4) with current AHPRA registration
  • Current First‑Aid certification
  • Demonstrated experience in community‑based clinical care and assessments
  • Strong understanding of aged care standards, clinical risk management, and incident response
  • Proven leadership skills and experience supervising or mentoring staff
  • Excellent communication, organisational, and interpersonal skills
  • Proficiency in clinical documentation and digital care systems
  • Valid driver’s licence and NDIS Worker Screening Clearance
  • Current COVID‑19 vaccinations with valid medical exemption if applicable
  • Influenza vaccination is recommended
  • Current National Police Check and Working with Children Check (WWCC)
  • Registered vehicle with minimum comprehensive insurance for transporting participants
