Kyabram District Health Service (KDHS) is a medium‑sized rural health service that delivers a range of programs and services including acute subacute residential aged care and allied and primary health care services. KDHS has three campuses located in the townships of Kyabram, Stanhope and Tongala.

the Role
The District Nursing Coordinator is responsible for facilitating a high quality, efficient and responsive home nursing and palliative care service to clients, carers and their families, empowering self‑management where appropriate. The Coordinator provides supervision and mentorship to the District Nursing team and can assist with clinical caseloads where required.
The Coordinator contributes to the strategic direction of the District Nursing programme in consultation with the Community Services Manager to ensure continual service improvement for the benefit of clients, carers and their families.
As a member of the District Nursing team, the Coordinator will provide leadership in achieving and upholding Kyabram District Health Service Purpose and Values.
About You
To be successful in this role you must be a registered nurse with a minimum of five years’ experience in a clinical environment and / or community setting, particularly in wound care, palliative care, gerontology, continence and community services, and an understanding of aged care issues. You must be self‑directed with good time‑management, communication and interpersonal skills.
You also need to demonstrate the ability to lead high‑quality care in a complex and changing environment, and confidence using information technology, including mobile devices.
