We are looking for dedicated Home Community Support Workers to provide high-quality, person-centered support to NDIS Participants. Your role will be crucial in helping them achieve their goals, enhance their quality of life, and improve their physical and mental health.

    • Key Responsibilities
    • As a Home Community Support Worker, your responsibilities will include :

    • Individualised Support
    • Providing a range of services tailored to meet the unique needs of NDIS Participants, such as housekeeping, grooming, toileting, meal preparation, medication management, and more.
    • Knowledge and Care
    • Demonstrating an understanding of health and well-being, cultural plans, transition plans, and behavior support plans. You will also be knowledgeable about protocols regarding restrictive practices and medication requirements.
    • Community Inclusion
    • Assisting Participants in participating in community activities and services based on their preferences and abilities. You will help them establish and maintain relationships and interests.
    • Communication
    • Providing regular feedback to the Home and Community Support Manager regarding issues that require follow-up. This includes matters related to Participants, their families, external supports, fellow employees, and community access activities.
    • Compliance
    • Adhering to UFC's Code of Conduct, policies, and Quality Management System procedures to ensure the highest standards of care and support.
    • Safety and Wellbeing
    • Demonstrating a strong commitment to the safety and well-being of yourself, Participants, and others in the work environment.
    • About You
    • You will thrive and succeed in our team by bringing the following experience to this position :

    • Certificate III or IV in individual supports (aged care, disability or home and community services)
    • Drivers License and vehicle
    • Experience working within the Home Care environment
    • Ability to meet deadlines, work with mínimal supervision and pressure.
    • Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
    • Understanding of WHS procedures
    • Desirable
    • Qualification in community services, disability, mental health or related field
    • Qualifications in nursing, assistant nursing or registered nurse
    • Manual Handling
    • Experience working with CALD clients
    • Complex care
    • Dementia care
    • Mental Health first aid
    • Certificate IV in mental health
    • Trauma informed care
    • Mental health awareness and sensitivity training
    • NDIS Worker Screening Clearance (obtained from Service NSW)
    • Current First Aid Certificate
    • Unrestricted Australian Driver's License and vehicle with Comprehensive Insurance
