Launnie Care Services is a NDIS provider committed to delivering high-quality, person-centered supports across Tasmania. We work alongside individuals with diverse needs, helping them build life skills, access services, and engage with their local communities. Launnie Care Services have established in July 2022 and since then provide support to people with Disability and making a difference in their live by providing consistency.

About the Role

As a Community Worker, you’ll be working directly with individuals, families, and groups to enhance community inclusion, social participation, and access to essential services. You’ll identify needs, coordinate support, and empower clients to achieve their goals through tailored programs and advocacy.

Description

Provide support, information and advice to clients on emotional, financial, recreational, health, housing and other social welfare matters, and evaluate and coordinate the services of welfare and community service agencies.

Tasks

  • Act as the first point of contact for new clients, families, and referrers.
  • Conduct initial risk assessments and complete intake documentation in accordance with the organisation’s process.
  • Conduct initial intake interviews to collect personal, medical, and psychosocial information.
  • Provide expert advice and support to prospective clients throughout the decision-making process, ensuring they are well-informed about service delivery.
  • Assess client needs, eligibility, and suitability for disability support services, including under the NDIS (National Disability Insurance Scheme).
  • Develop and document assessment reports and recommendations.
  • Collaborate with internal teams and external stakeholders to ensure timely access to appropriate supports.
  • Work closely with House Coordinators to explore suitable Supported Independent Living (SIL) options that align with the client's needs, preferences, and funding.
  • Maintain accurate records and perform data entry in client management systems.
  • Provide information, support, and guidance to clients regarding available services and processes.
  • Identify urgent or complex cases and escalate appropriately.
  • Uphold privacy, dignity, and cultural sensitivity in all client interactions.
  • Contribute to the continuous improvement of intake and assessment processes.
  • Identify and pursue new business opportunities through networking, attending industry events, and conducting market research.
  • Work collaboratively with the rostering and administration departments to ensure clients receive care and support aligned with their care plans and funded levels of support.

  • What We’re Looking For :

  • Qualification in Community Services , Social Work , or a related field (Diploma or higher)
  • Previous experience in a community or human services role
  • Excellent communication and problem-solving skills
  • Strong understanding of community development and social justice principles
  • Valid Driver’s License and reliable transport
  • NDIS Worker Screening Check and Working with Vulnerable People Check (or willing to obtain)
  • Please note : at least three years of relevant experience may be a substitute for the formal qualifications listed above. In some instances, relevant experience and / or on-the-job training may be required in addition to the formal qualification.
