Job descriptionPart of an established medical group with 130 staff across multiple locations Strong reputation in integrative and preventative healthcare On-site allied health including podiatry and counselling Located in a medical precinct with pharmacy, dental, pathology, and radiology Collaborative environment that values teamwork and clinical excellence Opportunity to work with a diverse range of patients, focusing on chronic disease management Dedicated and passionate CDM Nurse with a focus on preventative care Current registration as a Registered Nurse with AHPRA Proven experience in chronic disease management and care plan development Flexibility to work across multiple locations
