Positive Ageing - Program Coordinator

As the Positive Ageing Program Coordinator, will support customers to increase their connection with community in alignment with their individual goals to ensure that all program requirements are met as outlined by the Commonwealth Home Support Program (CHSP) grant funding agreement and organisational requirements.
Additionally, the Positive Ageing Program Coordinator is responsible for the delivery of objectives that promote living life to the fullest for older people by delivering and evaluating activities and programs that focus on social inclusion, client choice and wellness and reablement.
