About Us :

Co-owned and established in 2024 by three experienced Positive Behaviour Support Practitioners, Momentum 360 is a family-based business operating from rural Victoria.

We offer a wide range of services, including the development of Positive Behaviour Support plans, consultations, training for practitioners, and dedicated family support. Specialist Behaviour Support provided by Momentum 360 aims to enhance the quality of life for individuals who display behaviours of concern.

Our approach is firmly rooted in a strong commitment to human rights and person-centred practices, ensuring we create lasting momentum for individuals and families alike.

About You :

You are a values-driven professional who aligns with Momentum 360’s commitment to respect, integrity, collaboration, compassion, growth, and results. You bring established clinical skills and apply evidence-based, person-centred practice to support people with complex needs.

You hold a relevant tertiary qualification in Psychology, Behavioural Science, Social Work, Nursing, or an Allied Health discipline, and meet all professional and compliance requirements, including:

  • Rights to work in Australia
  • Current driver’s licence
  • Working With Children Check (WWCC)
  • NDIS Worker Screening Clearance

You work collaboratively with participants, families, and multidisciplinary teams and remain committed to continuous learning, supervision, and high-quality outcomes.

Position Summary :

The Positive Behaviour Support Practitioner provides specialist behavioural and therapeutic support to participants of Momentum 360. The role focuses on the design, implementation, and evaluation of evidence-based, person-centred interventions that reduce behaviours of concern and improve quality of life. Practitioners work collaboratively with multidisciplinary teams to ensure outcomes are ethical, measurable, and compliant with NDIS and legislative frameworks.

Key Requirements :

Essential

  • Registration registration as a Behaviour Support Practitioner with the NDIS Commission.
  • Demonstrated experience conducting functional behaviour assessments and developing BSPs.
  • Sound understanding of positive behaviour support principles and evidence-based practice.
  • Excellent written communication and report writing skills.
  • Works indendently and can problem solve independently
  • Demonstrated ability to work collaboratively within multidisciplinary and family-centred frameworks.

Desirable

  • Experience working within the NDIS environment.
  • Advanced data collection and analysis skills.
  • Training in trauma-informed care or neurodiversity-affirming practice.
  • Supervison skills

Job Responsibility :

Assessment and Planning

Conduct comprehensive functional behaviour assessments (FBA) and develop high-quality Behaviour Support Plans (BSPs).

Identify the function of behaviours and formulate proactive, reactive, and skill-building strategies.

Ensure all plans align with NDIS Quality and Safeguards Commission requirements, including restrictive practices protocols.

Implementation and Evaluation

Implement and review behaviour support interventions in collaboration with participants, families, and support teams.

Use data to monitor participant progress, measure outcomes, and adjust strategies to ensure continuous improvement.

Support the gradual reduction and elimination of restrictive practices in line with best-practice standards.

Collaboration and Communication

Work as part of a multidisciplinary team including psychologists, therapists, and support coordinators.

Liaise with families, carers, and external health professionals to ensure holistic care.

  • Prepare detailed clinical reports, progress notes, and NDIS documentation to professional standards.

Professional Practice and Safeguarding

Adhere to the NDIS Code of Conduct, the NDIS (Restrictive Practices and Behaviour Support) Rules, and Child Safe Standards.

Maintain participant confidentiality and act within ethical and legal boundaries.

Report incidents, risks, or potential harm through Momentum 360’s safeguarding procedures.

Capacity Building and Education

Provide training and guidance to support staff, families, and stakeholders in implementing positive behaviour strategies.

Mentor junior practitioners and contribute to a learning culture of clinical excellence.

Participate in supervision and professional development to maintain registration and enhance clinical capability.
