Job description

Position Description :

As the Mental Health Coordinator, you will design and deliver efficient and responsive community mental health nursing care and mental health case management, within the primary health care setting and model of care, in a professional and culturally safe manner, including but not limited to :

Working within the ways of an Aboriginal Community Controlled Health Organisation to provide health care services

Provision of holistic clinical care

Service and professional development

Relationship Management

Key Responsibilities :

Strengthening and maintaining culture in all service delivery ensuring culturally safe spaces

Empowering patients and families with self-determination to make their own decisions and take control of their health

Delivery of efficient and responsive mental health services to patients

Ensuring care is holistic encompassing social, emotional, mental, physical and cultural wellbeing of individuals and the whole community

Work within an integrated model of care to ensure a flexible responsive approach to services

Demonstrate clinical leadership within the multidisciplinary team to meet the changing needs of individuals and the community

Manage portfolio consistent with skill level and level of practice

Practice safety by demonstrating professionalism and accountability in comprehensive health promotion practices

Understand and address the unique needs of Elders

Monitor, evaluate and report outcomes of clinical services in line with PHN outcome measures e.g. MDS data extract

Key Requirements :

Bachelor of Nursing or equivalent is a mandatory qualification

Registration as a Registered Nurse under the Health Practitioner Regulation Agency {AHPRA} is essential

Registered Nurses working within Mental Health are to be credentialed through the College of Mental Health Nurses OR be prepared to work towards a credential in Mental Health nursing within an agreed period.

A Graduate Certificate or greater within a relevant specialty is highly desirable, OR in absence of this certificate, a commitment to completion of post-graduate study.

Working knowledge of health issues affecting Aboriginal and Torres Strait Islander people and a strong commitment to improving health outcomes

Experience in providing assessment, case management to Aboriginal and / or Torres Strait Islander people

Demonstrated skill in working in a primary care setting - in particular the ability to work as part of a multidisciplinary team with a sound understanding of your role in holistic ‘wrap around’ care for clients

Demonstrated effective verbal and written skills with particular emphasis on interpersonal communication skills
