Key Responsibilities :

  • Develop personalized support plans in collaboration with participants, focusing on their individual goals and aspirations.

Provide direct assistance to participants in various daily activities, including personal care, social engagement, and skill development.

  • Facilitate community integration and participation by organizing and accompanying participants to various events and activities.
  • Collaborate with families, carers, and other professionals to ensure holistic and coordinated support for participants.

  • Maintain accurate and up-to-date documentation in compliance with organizational standards and regulatory requirements.
  • Participate in ongoing training and professional development activities to enhance skills and knowledge in disability services.

    Essential Criteria :

  • Diploma qualification in Disability Services, Community Services, or a related field.
  • Demonstrated experience in supporting individuals with disabilities, including the development and implementation of individualized support plans.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with participants and their support networks.
  • Strong organizational abilities and the capacity to manage multiple tasks efficiently.

  • Commitment to promoting social inclusion and advocating for the rights and dignity of individuals with disabilities.
  • Valid driver's license and access to reliable transportation.

  • Current First Aid Certificate and Working with Children Check.
