Role Overview

The Registered Nurse – Community Nursing Programs is an office-based position that will assist in the delivery of in-home care for NDIS, Private and Brokerage and DVA clients. Key to the role is working with a team of Registered Nurse and Enrolled Nurses in case coordination through Pop-Up Health's software platform, Sandwai. Communicating clearly and transparently with the team, clients and their families you will imbed trust and cooperation.

Suitable candidates will be available to work shifts across a 7-day roster.

Key Duties & Responsibilities

• Packing wound boxes, unpacking stock and general maintenance of the consumables in the wound room.
• Providing general customer service by answering phone calls, complaint mitigation, problem solving, providing clinical advice within scope and escalating when required to Clinical Manager / Team Leader.
• Completing road shifts as required.
• Monitor complaints, incidents, and deteriorations in Sandwai with the wider team and ensure they are responded to in a timely manner – escalate as required to Clinical Manager.
• Complete client assessments as required.
• Provide clinical advice to road employees as required.
• Triage and monitor referrals through the referral inbox ensuring client onboarding documentation is completed in a timely and accurate manner.
• Create client service plans for home visits, develop an appropriate care plan and prepare documentation for initial visit.
• Liaise with existing clients and monitor feedback.
• Support field nurses with any queries or concerns.
