About the role

As the Clinical Nurse Specialist Grade 2 (Centralised Intake Coordinator), you will provide advanced clinical leadership to the community services intake function, ensuring timely, equitable and patient-centred access to care. Working collaboratively with multidisciplinary teams and key stakeholders, you will coordinate referrals, clinical triage and service access while supporting quality outcomes and operational efficiency.

Key Responsibilities

• Lead the centralised intake function, coordinating the referral, clinical triage, prioritisation and allocation of referrals to ensure timely access to community-based services.
• Manage service demand and waitlists by monitoring referral activity, prioritising care needs and reduce waiting times and optimise service capacity.
• Provide expert clinical leadership and decision-making, ensuring referrals are assessed and allocated in accordance with clinical need, service criteria and best practice standards.
• Collaborate with multidisciplinary teams, referrers, general practitioners, hospitals and community providers to facilitate coordinated, seamless and patient-centred care.
• Monitor, analyse and report on referral, waitlist and service activity data to identify trends, inform workforce planning and support continuous service improvement.
• Practice with a high degree of autonomy and accountability, demonstrating advanced nursing expertise, leadership and delegation in accordance with professional standards and organisational requirements.

About you

• Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
• Relevant post-registration qualifications and at least three years’ experience working in the clinical area of their specified post-graduate qualification
• Demonstrated commitment to ongoing professional development
• Demonstrated effective communication and interpersonal skills
• Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse
• Current unrestricted NSW Driver’s License
• Demonstrated ability to work within the interdisciplinary team at a senior level
