About the Role
As aCare Partner, youll be the vital link between our clients, their families, and our care team. Youll ensure that every client receives personalised, high-quality support tailored to their unique needs, helping them live safely and independently at home.
Key Responsibilities
Coordinate and schedule Support at Home services
Match clients with suitable care professionals based on skills and compatibility
Maintain regular communication with clients, families, and staff
Monitor and update client care plans and quarterly budgets
Ensure compliance with NDIS and Aged Care Quality Standards
Deliver exceptional customer service and maintain accurate records
About You
Minimum1 year of experiencein a similar Care Partner or Co-Ordination Role
Sound understanding ofNDIS and Aged Care Quality Standards(preferred)
Strong organisational, time management, and communication skills
Empathetic, client-focused approach to care
Confident using scheduling software and IT systems
Relevant qualifications in:
Certificate III or IV in Individual Support (Ageing or Disability), or
Diploma in Community Services
