An exciting opportunity to join the Gippsland, VIC region with Aspire4Life as a Clinical Assessor . This is a community-based role allowing you the opportunity to travel throughout the region to work collaboratively with clients both in their homes and in Residential Aged Care Facilities. You will use your clinical skills and knowledge to conduct assessments with residents and clients.

We offer both part-time and full-time permanent contracts , working Monday to Friday - no weekend or shift work required .

To ensure you feel confident and supported in your role, we provide a comprehensive two-week training and orientation program. The first week is in-person at our Coolangatta office with travel, accommodation and meals fully covered as per our travel policy. Our next orientation intake begins 28th April 2025.

The role :

  • Working independently within your region, utilising your assessment experience to conduct holistic style Home Care and Complex Aged care Assessments with a client centred approach focusing on wellness and reablement.
  • Capturing individual client information, working collaboratively to develop their support plan and unique goals.
  • Supporting older Australians to continue living independently, by considering supports such as assistive technology, social support services, and community care.
  • Travel to Aged Care Facilities to conduct Residential Aged Care Funding Assessments.
  • Conduct assessments using a range of assessment tools within the My Aged Care portal.

    • About you :

  • Experience in assessment and goal orientated support planning in aged, community, health or social services.
  • Confident communicator with a positive approach when collaborating with clients, families, staff, and other stakeholders.
  • Local knowledge and the ability to network with stakeholders within your region.
  • Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite).
  • Previous experience working with KPI's to ensure high quality client outcomes.
  • Ability to manage your time effectively in an autonomous working environment.

    • What to bring?

  • Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with 5+ years of experience.
  • Minimum 5 years' experience within aged care.
  • Evidence of an influenza vaccination (for the 2024 Flu Season).
  • Evidence of COVID-19 Vaccinations (minimum two doses + Booster).
