Team Leader – Aged Care

• Experience working with Indigenous and culturally diverse communities
• Knowledge of NDIS frameworks and aged care funding models
• Demonstrated experience in aged care service coordination or leadership
• Proven ability to mentor and support a team of care workers
• Excellent communication and interpersonal skills
• Current NT Drivers License Ochre Card Criminal History Check NDIS Worker Screening Clearance First Aid & CPR Certification.

Desirable Criteria

• Knowledge of CHSP guidelines My Aged Care portal and aged care compliance
• Experience in care planning client assessments and service delivery
• Proficiency in CRM systems Microsoft Office and aged care reporting
• Ability to manage remote service delivery and liaise with external providers
• Tertiary qualifications in Aged Care Home Care Disability or a related field
• Experience working with Indigenous and culturally diverse communities
