As an Intensive Case Manager you will deliver the Journey to Social Inclusion (J2SI) program providing long-term personalised support and case management to people experiencing chronic homelessness. Working with a small caseload youll use a trauma-informed team-based approach to help clients access housing build life skills and foster independence.
Our work isnt easy but it matters. Work with people who care as much as you.
