About the role

Reporting to the Triage & Navigation Manager, the Supportive Care Navigator is a key member of our Health Services team. You will be the first point of contact for many clients and healthcare professionals seeking support, managing inbound referrals and conducting comprehensive assessments to understand individual needs. Working across phone and digital platforms, you will provide information, emotional support and service navigation to help people affected by blood cancer access the most appropriate supports, resources and services. We welcome applications from candidates located near any of our offices across Australia, as this role can be based in any of our locations nationwide. This role is funded through the Australian Cancer Nursing and Navigation Program (ACNNP)

What You'll Be Doing

• Manage inbound referrals and enquiries from clients, families and healthcare professionals
• Conduct supportive care needs assessments and risk assessments
• Provide accurate information about blood cancer and available support services
• Coordinate access to Leukaemia Foundation services, including practical, emotional and digital support options
• Facilitate internal and external referrals where required
• Maintain accurate and compliant client records and documentation
• Collaborate with healthcare professionals and multidisciplinary teams
• Contribute to quality improvement initiatives and best-practice service delivery
• Support positive client experiences and outcomes throughout the blood cancer journey

What You'll Bring

• Tertiary qualification in Social Work, Nursing or an Allied Health profession
• Current AHPRA registration or membership with the relevant professional body
• Strong understanding of supportive care principles and person-centred practice
• Excellent communication and interpersonal skills, particularly when supporting people through sensitive situations
• Experience conducting assessments and coordinating care or support services
• Strong organisational and time management skills
• Confidence using technology and maintaining accurate client records
• Ability to work both independently and collaboratively within a multidisciplinary team
• Emotional resilience and a commitment to achieving positive client outcomes
