Community Support Worker Location: Sydney, NSW Employment Type: Full-time Hours: 38 hours per week Salary: $85,000 per annum + superannuation About Image Home Healthcare NSW Image Home Healthcare NSW provides personalised disability and NDIS support to help participants build independence, confidence, wellbeing and community connection. We work closely with participants, families, support coordinators and other service providers to develop flexible support that reflects each person’s individual goals, preferences and circumstances.

We’re growing, and we’re looking for full-time Community Workers who want to help our clients build independence, confidence and a real sense of belonging. About the role As a Community Worker, you will assess participant needs, coordinate appropriate services, develop and implement support programs, encourage community participation and provide practical assistance with daily living and personal goals.

This role is suited to someone who can combine strong interpersonal skills with sound case coordination, community engagement, documentation and support-program skills. Key Responsibilities

  • Assess clients' individual needs and circumstances through home visits, consultations and ongoing communication, identifying their social, emotional, health, welfare and daily living support needs.
  • Develop and implement individualised care and support plans based on clients' assessed needs, preferences and personal goals.
  • Interview and consult with clients and their families or carers to identify difficulties affecting clients' wellbeing, independence and participation in everyday and community activities.
  • Monitor and regularly review clients' progress and changing circumstances, evaluate the effectiveness of existing support arrangements, and recommend appropriate adjustments to individual care and support plans.
  • Coordinate the delivery of home and community-based support services in accordance with individual care plans and ensure services meet clients' assessed needs and agreed support goals.
  • Support clients to maintain social connections and participate in appropriate community activities, promoting independence, confidence and social inclusion.
  • Provide clients and their families or carers with information and guidance regarding available home-care services, support arrangements and service options.
  • Maintain accurate client records, assessments, case notes, care plans and progress reports and appropriately document changes in clients' circumstances and support requirements.
  • Identify concerns relating to clients' safety, wellbeing or ability to live independently at home and communicate relevant concerns to management and responsible members of the care team.
  • Contribute to the ongoing review and improvement of home-care programs and service delivery by identifying changing client needs, service gaps and opportunities to improve client outcomes.

Skills and Experience The successful candidate will have:

  • An AQF Certificate IV or higher qualification in Community Services, Aged Care, Disability Services, Individual Support, Social Welfare or a closely related field.
  • Relevant experience in community services, home care, aged care, disability services or a related welfare service environment.
  • Demonstrated ability to assess client needs and develop and implement individualised care and support plans.
  • Experience monitoring client wellbeing, progress and changing support requirements.
  • Strong interpersonal and communication skills and the ability to communicate effectively and sensitively with clients, families and carers from diverse backgrounds.
  • Good organisational, problem-solving and time-management skills.
  • Sound understanding of person-centred care, client dignity, independence, privacy, safeguarding and professional boundaries.
  • Ability to prepare and maintain accurate client assessments, case notes, care plans and progress documentation.
  • Competency in electronic client-management systems and standard office software.

About You You are a compassionate and organised community-services professional who understands the importance of person-centred support. You are able to assess individual circumstances, identify changing support needs and work collaboratively with clients and their families to achieve positive outcomes. You are committed to helping clients maintain their independence, wellbeing and connection with their communities while receiving appropriate support in their own homes.
