About the Role

\n

The Home Therapy Unit provides assessment, training, education and ongoing support for patients undertaking both Haemodialysis and Peritoneal Dialysis at Home.

\n

\n

An opportunity currently exists for Grade 2 Registered Nurses to join the Dialysis Home Therapies Team. As a Grade 2 Registered Nurse you will be responsible for the delivery of expert nursing care and support to patients undergoing Haemodialysis or Peritoneal Dialysis at home. Additionally, you will specifically review current assessment and training practices within the Home Therapies Unit and develop tools and KPI’s to effectively assist nursing staff.

Classifications for this position will range from YU11 ($102,278.80 per annum) (pro-rata as applicable).

Duties

\n

\n
• \n

  Patient assessment.

\n
• \n

  Patient and family Education and training

\n
• \n

  Home visiting for training and Assessment

\n
• \n

  Document, observe and report relevant nursing findings about patients’ status’ and progress

\n
• \n

  Liaise with patients, patient’s family, medical and ancillary staff as required

\n
• \n

  Acts as a role model and provide support to junior staff in all clinical aspects

\n
• \n

  Actively participate and contribute to all departmental quality improvements

\n

\n

Qualifications, Skills and Experience

\n

\n
• \n

  Minimum 24 months previous experience in Renal Nursing and/or experience with Peritoneal Dialysis essential

\n
• \n

  Minimum 12 months experience in Haemodialysis

\n
• \n

  Experience in patient and staff education

\n
• \n

  Proficiency in clinical nursing assessment skills

\n
• \n

  Ability to work autonomously prioritise tasks and activities to foster good time management

\n
• \n

  Operate within a “patient centred care” framework

\n
• \n

  Facilitate team outcomes in a multidisciplinary environment

\n
• \n

  Assist and support change and implement process improvements

\n
• \n

  Communicate effectively, with patients and staff at all levels

\n
• \n

  Computer Literacy- Microsoft word and dialysis user applications

\n

\n

St Vincent’s Hospital Nephrology Department provides care for patients across the entire spectrum of renal disease, from the ambulatory care of those with chronic kidney disease, to meeting the dialysis and transplant needs of those with End Stage Kidney Disease.

SVHM is known as a hub provider of renal services supporting a number of regional and rural satellite as well as Home Dialysis and outpatient dialysis facilities at Fitzroy and Kew.
