Make a Real Difference in Mental Health Recovery An established and respected community mental health provider is seeking experienced Clinical Care Coordinators to join its multidisciplinary team in Rockhampton and Hervey Bay.

This rewarding opportunity supports individuals living with severe, persistent and complex mental health conditions who require coordinated, multidisciplinary support beyond standard primary care. You'll play a vital role in delivering person-centred care that promotes recovery, wellbeing and long-term positive outcomes.

The Role As a Clinical Care Coordinator, you will:

- \n
- Manage a clinical caseload and provide direct mental health support \n
- Conduct comprehensive assessments, case formulations and care planning \n
- Deliver evidence-based therapeutic interventions \n
- Undertake risk assessments and develop risk management plans \n
- Coordinate care across clients, families, healthcare providers and community services \n
- Facilitate referrals and support transitions between services \n
- Develop strong community and stakeholder relationships \n
- Contribute to clinical governance, reporting and quality improvement initiatives \n

About You
\n
You are a compassionate and experienced mental health professional who is passionate about recovery-focused care.
\n
You'll bring:
\n
- \n
- Experience supporting people with complex mental health presentations \n
- Strong understanding of community mental health services \n
- Trauma-informed and recovery-oriented practice experience \n
- Excellent communication and relationship-building skills \n
- Ability to work effectively within multidisciplinary teams \n
- A person-centred, non-judgemental and culturally responsive approach \n

Essential Requirements
\n
- \n
- Relevant tertiary qualification in:\n
- \n
- Psychology \n
- Social Work \n
- Mental Health Nursing \n
- Occupational Therapy \n
- Or a related mental health discipline \n
\n - Current professional registration or eligibility relevant to your discipline \n
- Current Australian driver's licence \n
- National Police Check \n
- Working With Children Check (or applicable exemption) \n
- Full Australian working rights \n
