About the Department / Program

Monash Health Pharmacy is a diverse and dynamic team of over 500 professionals. We work across seven hospitals and community settings, delivering best-practice pharmacy services and supporting career development through structured rotations, mentoring, and leadership opportunities.

About the Role

Join our team as a General Medicine Pharmacist on a part-time (0.4 EFT), fixed-term basis until July 2027, based at Dandenong Hospital.

Working alongside the General Medicine Pharmacy Team Leader and broader multidisciplinary team, you’ll deliver expert clinical pharmacy services while contributing to education, quality improvement and the ongoing development of General Medicine pharmacy practice.

Key responsibilities include

• Collaborate with the General Medicine Team Leader to monitor clinical pharmacy services within General Medicine at Monash Medical Centre, ensuring activity, quality, safety and accreditation targets are achieved.
• Collaborate with General Medicine Pharmacy Team Leaders across Monash Health to standardise practices, develop clinical guidelines, procedures and patient information resources.
• Support clinical governance initiatives, including the development of competency frameworks, role requirements and workforce capability strategies for pharmacy staff.
• Support oversight of the Partnered Pharmacist Medication Charting (PPMC) program, including governance, activity monitoring, training and credentialing of pharmacists.
• Support the implementation, monitoring, training and credentialing activities associated with Pharmacist-led Discharge Prescription Planning (PDPP) to support safe and effective patient care transitions.
• Work flexibly across all sites as required, including travel to other Monash Health locations
  

About You

We’re looking for a pharmacist who is passionate about clinical practice, enjoys working as part of a multidisciplinary team and is committed to improving patient outcomes.

You will bring:

• Demonstrated leadership experience with the ability to coach, mentor and develop high-performing teams
• Extensive hospital pharmacy experience, ideally with General Medicine exposure
• Demonstrated ability to work effectively in fast-paced, complex clinical environments
• Strong communication, stakeholder engagement and collaboration skills
• Experience in clinical governance, quality improvement and medication safety
• Excellent organisational, decision-making and problem-solving skills
  

Mandatory requirements of the role

• Registered with the Australian Health Practitioners Agency (AHPRA) Pharmacy Board
• Relevant post graduate qualifications (desirable)
• Clinical Supervision training (desirable)
