Post Acute Care - Registered Nurse (HITH/PAC Level 1)

About the Role

This is a 6 month Fixed Term Full time position until 14th March 2027 at 80 hours per fortnight within the Complex Care Hub Post Acute Care program.

The Post Acute Care (PAC) Service forms part of the Complex Care Hub and is a statewide, short-term, multidisciplinary service supporting children and young people at home following an acute hospital admission or Emergency Department presentation. The program goal of the service is to assist tranitioning children and their families from the hospital bact to a home environment. It aims to enhance and maintain the independence of both the patient and their caregiver, ensuring that the child can continue theri recovery and care in the comfort of their home. The program provides vital support to ease the challenges of adjusting to post-hospital care, promoting a seamless and sato improve and maintain a patient/carer's independence within the community, supporting a smooth transition from inpatient setting to home environment.

This role involves close collaboration with the multidisciplinary team to educate patients and families, on organisational processes which support discharge and link with community supports.

The purpose of the Post Acute Care Registered Nurse role is:

• To provide family centred care through a holistic approach using systematic assessment of child’s medical needs in conjunction with their psychosocial, cultural and spiritual needs
• To plan and implement nursing care for patients in the Post-acute Care service.
• Provide coordinated care and clinical support to enable safe transitions from hospital to home, improve health outcomes, and prevent unnecessary hospital readmissions.

Classification for this position is at Yu15 (FTE base salary $115,289.20 per annum, plus superannuation).

Your Skills and Experience

• Demonstrated advance clinical knowledge and expertise in Peadiatrics
• Proven ability to apply advance clincial reasoning and critical thinking skills to complex patient care situations
• Ability to critically appraise research findings and apply evidence- based practices to improve patient care and outcomes
• Proven ability to collaborate effectively with interdisciplinary teams, fostering a culture of teamwork and shared responsibility for patient care.
